Gym HQ Limited – Junior Member Safety Release & Liability Waiver

This agreement applies to junior members aged 12–15 years using Gym HQ Limited, Tenbury Wells.
This waiver must be completed and signed by a parent or legal guardian.

1. Assumption of Risk

I understand that participation in physical exercise and the use of gym equipment at Gym HQ Limited involves inherent risks, including but not limited to:

Muscle strains and tears
Sprains, fractures, or other injuries
Cardiac or respiratory complications
Slips, trips, and falls

I voluntarily consent to the junior member participating and accept full responsibility for any risks, injuries, or damages that may occur.

2. Health Declaration

I confirm that:

The junior member is physically fit to take part in physical activity
A PAR-Q / health questionnaire has been completed honestly and accurately on their behalf
Any medical condition, injury, or limitation has been disclosed
I understand the junior member must stop exercising immediately if they feel pain, dizziness, or discomfort

3. Release of Liability

To the fullest extent permitted by UK law, I agree to release, waive, and discharge Gym HQ Limited, its directors, owners, employees, contractors, and representatives from any liability for:

Injury, illness, or death
Loss or damage to personal property

Except where such liability arises from negligence that cannot legally be excluded under UK law.

4. Junior Member Rules & Supervision

I understand and agree that:

Junior members must be supervised at all times by a responsible parent or legal guardian
Junior members are not permitted to use the gym during unstaffed hours
Access to certain equipment and training areas may be restricted
Gym rules, signage, and staff instructions must be followed at all times

Failure to comply may result in suspension or termination of membership.

5. Use of Equipment & Facilities

I accept responsibility for ensuring that the junior member:

Uses equipment correctly and safely
Does not use equipment that appears damaged or unsafe
Cleans equipment after use

6. CCTV Acknowledgement

I understand and accept that CCTV is in operation throughout Gym HQ Limited for the purposes of safety, security, and crime prevention.
Footage is stored securely and managed in accordance with UK GDPR and data protection laws.

7. Personal Belongings

I understand that Gym HQ Limited is not responsible for loss, theft, or damage to personal belongings brought onto the premises by the junior member.
